Step 1: Schedule Your Evaluation
You can schedule your initial evaluation in the way that works best for you:

Once your appointment is scheduled, a member of our team will reach out to gather any additional information needed prior to your visit.

You’ll be connected with Lauren, who will:

Step 2: Obtain a Physician Referral
Please have your physician fax a referral to: 608-621-3804
A referral is required prior to your evaluation appointment.
Step 3: Submit Insurance Information
To verify your benefits, please email a copy of your insurance card (front and back) to:
